In a rematch of last year's final, Spain's Carlos Alcaraz faced a tough challenge against German 23rd seed Jan-Lennard Struff. Alcaraz missed out on four match points at 5-3, leading to a deciding set tie-break.
However, the world number three managed to win in front of his home crowd, wrapping up a 6-3 6-7 (5-7) 7-6 (7-4) victory after nearly three hours.
Alcaraz, who won the previous two editions of the Madrid Open, is now on a 14-match winning streak in the tournament.
Next, the 20-year-old will face Russian seventh seed Andrey Rublev, who defeated Dutchman Tallon Griekspoor 6-2 6-4 earlier in the day.
Additionally, Italian top seed Jannik Sinner advanced to the quarter-finals after beating Karen Khachanov 5-7 6-3 6-3. He will face either Casper Ruud or Felix Auger-Aliassime in the next round.
In other matches, Daniil Medvedev defeated Alexander Bublik 7-6 (7-3) 6-4, setting up a clash against Rafael Nadal or Jiri Lehecka. German fourth seed Alexander Zverev, the 2021 champion, suffered a 6-3 6-4 defeat against Francisco Cerundolo.
